Medical Equipment Program
Any person receiving Medicaid and/or Medicare and requires medical equipment, can
order that equipment through Mr. Otis J. Williams. You can complete a Medical
Equipment Requirement Form (Adobe PDF format) through the New Beginnings web
site and print it on your printer. All forms are to be faxed directly to Mr. Williams. He will
order the medical equipment. New Beginnings does not handle these forms directly.
Below, are listed some of the available items (for a complete listing see form). Keep in
mind that both Medicaid and Medicare must review these submissions before signing
off and authorizing your equipment. In some cases, some of the items can only be paid
for by Medicare – in other cases only Medicaid. To save your valuable time, submit your
order, and let Mr. Williams separate which agency pays for what.
If you are enrolled into a Home Health Care Program, the medical physician attached
to the Home Health Care Program must sign for any medical equipment that you need
and he/she must write the requisite prescription. He/she must then fax the prescription
and completed form to Mr. Williams at 1-312-277-8763.
If you are not enrolled into a Home Health Care Program, we will recommend you to an
agency that will handle enrolling you into a Home Health Care Program. The Home
Health Care Program will provide a physician, therapist and nurse for home visits.
Ordering Information – When submitting your completed forms, it is important that all of
the information be entered into the forms correctly. Incorrect submissions take a lot of

persons helping him can be volunteers. He is permitted to have an agency such as New Beginnings help him secure persons who need medical
equipment. In this instance, New Beginnings acts in a volunteer capacity and will refer all requests directly to Mr. Williams. New Beginnings
receives no compensation in these transactions. These are Medicare Rules and Regulations.

- Submit the MERS Form to the Primary Care Physician (PCP) for his/her
signature and approval; - Request that the PCP fax the completed form along with a prescription to Mr.
Williams at 1-312-277-8763. - The forms can be filled out on the computer and printed prior to submittal to your
Primary Care Physician for clarity. Forms must have an original signature.
