Health Care

Medical examinations for residents seeking to
obtain drug treatment and counseling;

Medical screening for TB, HIV, AIDS,
hypertension is performed by Circle Family Care’s Mobile Health
Teams;

Medical examination for dental care,
eyeglasses

Medical and psychiatric examination for
Social Security benefits;
Department of Human Services (DHS/IDPA)
Assist clients and residents to
the following services:

DHS/IDPA (medical card, food stamps and
financial assistance) benefits;
-
Obtain assistance through special projects
for furniture, appliances, etc;

Obtain legal representation through the Legal
Assistance Foundation;
Department of Children and Family
Services
Assist IDOC “DCFS Wards” coming from the penal
institutions to access the following services:
-
State identification cards, Social Security
cards, birth certificates, voters registration cards, school
transcripts and such other items as may be needed to help the
client to obtain his state ID;

Prior to establishing a linkage with DCFS for
individual clients, we assist clients to obtain clothing, shoes,
coat, etc;
-
Referrals are provided to appropriate
schools, continuing adult education programs, job readiness, job
training and/or job placement programs;
-
Referrals are provided for medical and dental
services as needed;
-
Assessments for various mental
health/behavior modification services are also provided on an as
needed basis;
Outpatient Behavior Modification
Counseling
-
Individual and Group Behavior Modification
Counseling is offered through Diagnostic Behavioral Services;
-
Psychiatric counseling is done through
agencies such as Bobby Wright Comprehensive Mental Health
Center, Loretto Crisis Management Center and Triage Program;
-
Drug Treatment and Counseling is done through
Habilitative Systems, Healthcare Alternative Systems and
Garfield Counseling Center;
Legal Assistance
-
Assist clients and residents who have
violated their parole or probation to re-establish contact with
their probation officer or parole agent;
-
Assist clients and residents to locate a
Legal Assistance Attorney to represent them in court, Social
Security hearing or administrative review;
-
Assist clients and residents by appearing in
court on behalf of our agency to explain how our agency works
and how we can benefit persons that would otherwise be sent to
jail or the penitentiary.
HIV/AIDS Education Programs
-
Assist HIV/AIDS clients to access services
through the Core Center;
-
Assist HIV/AIDS clients to obtain the name,
address and phone number of their external case manager;
-
Assign an Internal Case Manager to assist
HIV/AIDS clients to obtain a variety of pre-identified services;
-
Enroll IDOC and CCDOC HIV/AIDS clients into
the Chicago Department of Public Health
– Department of
Corrections Initiative;
Jobs Unlimited Program
-
Assist clients and residents to locate
gainful employment;
-
Arrange for clients and residents to be
transported to and from jobsites that are inaccessible to public
transportation;
-
Assist clients and residents to access
on-the-job training programs;
-
Assist clients and residents to enter job
readiness and placement programs;
-
Assist clients and residents to obtain
clothing, tools, shoes etc. through local Welfare-to-Work
Program Initiatives;
Housing
-
Provide intensive 1st stage
transition/recovery type housing for chemically dependent,
moderately mentally challenged, HIV/AIDS impacted and
ex-offenders;
-
Provide restricted, monitored, drug and
alcohol free 2nd stage housing for those residents
who have completed a minimum of 200 consecutive clean and sober
days, employed, and fulfilling the rules and regulations
incumbent upon persons in this status;

Provide access to homeowners who are willing
to rent apartments to our clients;
Social Security
-
Assist clients/residents to access Social
Security to obtain Social Security benefits;
-
Assist clients/residents to complete
vocational and/or disability report forms;
-
Assist client/residents to complete
Activities Questionnaire Form;
-
Assist clients/residents to obtain legal
representation through an SSI approved attorney;
-
Assist clients/residents to complete
Reconsideration Filings and assist in the preparation of such
reports, or the obtaining of medical reports from other agencies
as a means of aiding the client secure SSI benefits;
